It’s easy! Create an account and schedule your first visit with your care management team. Your personalized care management plan will get delivered right to your Vori Health account.
Depending on what we learn about you, your goals, and your needs from your initial assessment, our care management team will provide a personalized care management plan that can include at-home exercises, in-person physical therapy, nutrition counseling, and healthy lifestyle coaching.
Typically, patients meet virtually with their care management team one to two times per week during the initial stages of the care management plan. And to help you stick to it and achieve your goals, you’ll get the support and guidance of a personal health coach. You can also message your care management team at any time. Together we’ll regularly review your progress and adjust your care management plan as needed.
Your Vori Health care management team is personalized to your needs and may include board-certified specialty medical doctors and nurse practitioners, as well as care managers who have training in physical therapy, health coaching, and nutrition.

Some Vori Health members begin to see and feel changes in as little as a few weeks.2 Each person’s progress differs, but your care management team will be there every step of the way to help you achieve your goals and live pain free.
Vori Health is designed for convenience and effectiveness, so you can connect with your care management team by video, phone, or chat; you can do the exercises on your own or with your care management team, all from the comfort of your home. The majority of our members are able to achieve their goals using this method.2 However, if your care management team feels that you would benefit from an in-person visit, we also offer access to in-person physical therapy.

Our care management teams frequently partner with other doctors and physical therapists to make sure people like you get the best care possible. We can address any unresolved issues you may have and can work with your current care management team.
